Transition support can include skill-building sessions covering daily living skills, communication and decision-making, along with planning and mentoring to prepare for what's ahead. It often involves coordinating with schools, employers or other service providers, and can run over a short, defined period or longer term depending on the transition. Big life changes are harder to manage without a plan, and structured support during a transition builds the confidence and practical skills a participant needs to make the change stick.
We work with the participant and their family to map out the transition ahead of time, connect with schools, employers or Supported Independent Living providers where relevant, and build toward a gradual handover of responsibility as confidence grows. This support is delivered across Brisbane, Logan, Ipswich and the Gold Coast.
